A short time ago while doing home-health "opens" for a nationally-known company, I would fill out the consent forms, get the 6 signatures from the patient, list all the medications on a company form down to the milligram, then pull out the mother of all forms - - - , the OASIS. Q: Why do so many PT companies require so much paperwork"
A: THIRD PARTY PAYERS!

Back in Scottsdale twelve years ago I had an involved Parkinson's client who wanted help to go in his pool. As medicare would no longer pay for that I charged $60/30 minutes (a fictitious time as I always went over). When I stated I would need to write a note, the client's sweet wife said, "I'm not going to pay for a NOTE". To see pictures browse the www.AtHomeMEDrehab.com site and you'll find it.


If clients were paying the bill directly who could afford to pay for two and a half hours of paperwork? Even if they could afford it, who in his or her right mind would pay for it? Having participated in the 3rd party payer "dance" I have come to realize how easy it is to let 3rd party payers control us. If the client may indeed benefit from poorly reimbursed modalities when acutely injured but the 3rd party reimburses us less than the cost how easy it is to leave out the ultrasound and cold packs, take less time to listen to the patient, and rush them to exercise. It is tempting to use up the allowed number of visits when in fact the client could be getting most of the benefit from a lesser frequency and fewer visits?


Q: How do clients value our services when they expect to get them for FREE?
A: How many clients who are willing to pay cash for services that are "uncovered"?

How many visits have we done for which the patients pay cash at the time of service? I'm sure it's less that the number that were rendered for "free". FREE is the highest price. I am convinced that we must become able to offer such incredible value that our clients are willing and eager to pay out of pocket in order to receive them.
